Buffalo City Mission Referral Portal

Referral Overview

Our goal in the hospital referral process is to keep our clients safe during their care transition and assist with efficient hospital discharge.  Please follow the steps listed below to ensure a complete referral.  All incomplete referrals will be returned without full review.

1. Determine client eligibility for referral

– Review the Buffalo City Mission Emergency Shelter 4H Eligibility Tool
– If every answer to every question is “Yes” proceed to completion of the referral

2. Determine timing of admission

– Referrals and Admissions can be received 24/7
– Referral responses will be communicated by fax, phone or email within 120 minutes
– Referrals can be made up to 72 hours prior to expected admission
– New referral required if admission does not take place in 72 hours

3. Complete all sections of the BCM Emergency Shelter Hospital Referral Form and compile required documents

ALL INCOMPLETE REFERRALS WILL BE RETURNED WITHOUT FULL REVIEW

4. Wait for Referral Decision from BCM

DO NOT DISCHARGE THE CLIENT WITHOUT BCM REFERRAL DECISION CONFIRMATION
